Globe Calm Health Center Disclosure Statement/ Services Document
Thank you for choosing Globe Calm! We are elated to have the opportunity to be working with you on your journey to becoming a better you. This document is highly encouraged to be read to the end as it clearly defines our policies as a counseling practice. Therapy is a relationship that works in part because of clearly defined rights and responsibilities held by each person. This fosters a safe environment to explore and take risks to encourage change. As a client in Globe Calm, you have certain rights that are important for you to know because this is your therapy, whose goal is your well-being. There are certain limitations to those rights that you should be aware of.
You have the right to ask questions about anything that happens in therapy. We are always willing to discuss how and why we’ve decided to do what we’re doing, and to look at alternatives that might work better. You can feel free to ask us to try something that you think will be helpful. You can ask us about our training for working with your concerns and can request that we refer you to someone else if you decide we are not the right therapists for you. You are free to leave therapy at any time.

Your Responsibility as a Client
You are responsible for coming to your session on time and at the time we have scheduled. Sessions last for 50 minutes. If you are late, we will end on time and not run over into the next person’s session. If you miss a session without cancelling or cancel less than twenty-four hours’ notice, you must pay for that session at our next regularly scheduled meeting. The only exception to this rule is if there is a huge emergency. If you no-show for two sessions consecutively and do not respond to our attempts to reschedule, we will assume you have dropped out of therapy and will make the space available to another individual.
Payment for Service: Payment may be made by cash or credit card
You are responsible for paying for your session weekly unless we have made other firm arrangement in advance. If we decide to meet for a longer session, we will bill you prorated on the hourly fee. Emergency phone calls of less than ten minutes are normally free. However, if we spend more than 10 minutes in a week on the phone, responding and reading emails, we will bill you on a prorated basis for that time. Kindly note we do have operating hours and may not respond after 5PM or if engaged in any personal or business-related activity. In this case leave a message and we will respond when available.
Payments for online sessions must be paid 2 days before the session.
Cancellation: (within 24 hours of appointment) or no-shows: $100
Our Responsibilities to You as Your Therapists
Confidentiality
With the exemption of specific exceptions described below, you have the absolute right to confidentiality of your therapy. We cannot and will not tell anyone else what you have told us, or even that you are in therapy without your prior written permission. We can only release information to others if you sign a written Authorization form. There are other circumstances that require only that you provide written consent. We will always act to protect your privacy. You may direct us to share information with whomever you chose, and you can change your mind and revoke that permission at any time. You may request anyone you wish to attend a therapy session with you. You should also know that it is very helpful to consult with another mental health professional about your case. During a consultation, the identity of a client is completely anonymous, and the other mental health professional is ethically bound to keeping the information confidential.
If you chose to communicate with us by email, messenger, WhatsApp or any application at some point in our work together, please be aware that these portals of communication are not completely confidential. All information from the electronic applications are retained in the logs of your or our internet service provider. While under normal circumstances no one looks at these logs, they are, in theory, available to be read by the system administrator of the internet service provider. Any email we receive from you and any responses that we send to you, will be kept in your treatment record.
The following are exceptions:
The next is not a legal exception to your confidentiality. However, it is a policy you should be aware of if you are in couples therapy with us.
If you and your partner decide to have some individual sessions as part of couples therapy, what you say in those individual sessions will be considered to be part of the couples therapy, and can and probably will be discussed in our joint session. Kindly not tell us anything if you wish kept secret from your partner. We will remind you of this policy before beginning such individual sessions.
Diagnosis
If a third party such as an insurance company is paying for part of your bill, we are normally required to give a diagnosis to that third party to be paid. Diagnoses are technical terms that describe the nature of your problems and something about whether they are short-term or long-term problems. If we do use a diagnosis, we will discuss it with you. All of the diagnoses come from a book titled the DSM-V; we have a copy in our office and will be glad to let you borrow it and learn more about what it says about your diagnosis.
